I had someone request a return on amazon.ca and never had this happen before. Do I need to do anything? Do I get dinged for shipping costs back to me?

You have to accept the return or buyer can file a A-Z claim. Buyer is responsible for shipping it back to you. After you receive the item and is in the same condition as you send it, then you issue a full refund less shipping fee collected.

That's if they return it. I approved a return request and never recived the item back. Tried emailing the buyer but never got an reply back. Odd 

If they don't return it, you don't need to refund the money right?

I don't think so but if you don't approve the return, Amazon can approve it without you & then the customer isn't necessarily required to return it..

Never issue a refund without reviving the merchandise back
